About the Role:
At BILH - Addison Gilbert Hospital’s GHS-SBHC, the Community Health Worker reflects the values of the GHS-SBHC while forming connections with the GHS student community. The CHW works primarily with Gloucester High School Students and families to make sure they have access to healthcare and health resources, and works to remove any barriers to access. In this role, the CHW collaborates with GHS Staff and Community Partners to better support students/families. For this reason, the following skills are highly valued:
Excellent communication skills - both verbal and written; active listening skills with teens and adults
Strong Organizational Skills to manage and prioritize work
Established computer literacy skills - Microsoft word; Excel; google docs/forms/sheets/slides etc.
EPIC medical record registration experience is a plus
Bilingual (or multilingual) in English and Spanish or Portuguese is essential
Compassionate and emotionally resilient
Responsibilities:
Help create a safe and welcoming environment for all
Register CHW visits through EPIC
Assist with all areas of the Free Food Locker Shelf Pantry - organization, track and report for in-school snacks and food orders for home, help with home deliveries, etc.
Co-Advise Walking Club
Promote racial and social equity through participation in SBHC and GHS student programs and in work with EL students
Collaboration with GHS/Community Partners to connect students to services and attend community events
Complete home visits as needed
Assist with running student support groups as needed
Attend programs & training as required for BILH and MDPH
Promoting health and wellness in the student population by participating in and carrying out administration of public health initiatives, including representing the GHS-SBHC at community events
Cultural sensitivity and existing cultural connection required
Provide culturally appropriate health education, information, outreach
Assist in the development and maintenance of up-to-date materials, including website
Provide direct services, such as informal counseling, social support, care coordination, to include screening for social determinants of health
Organize GHS classroom visits to promote GHS-SBHC
Training as required to receive and maintain CHW certification
Support Program Manager as needed for MDPH reporting
Education - Experience - Requirements:
Education High School or GED minimum
Experience -
Technology- the ability to use computer programs with ease
CHW certification required within first 6-12 months of employment
CHW experience is a plus
Experience in a health care and/or school setting is a plus
Knowledge of Electronic Medical Records registration - EPIC, etc. is a plus
Work location is in Gloucester High School - living in or near Gloucester and having an understanding of the student community population we serve is a plus, but not required
Legally able to work in the US - ie US citizenship or have a work permit
Beth Israel Lahey Health - Addison Gilbert Hospital’s Gloucester High School - School-Based Health Center (GHS-SBHC) is a school-based teen health clinic that is funded by AGH and a grant from the Massachusetts Department of Public Health. The SBHC is staffed with a Program Manager, Nurse Practitioner, Licensed Independent Clinical Social Worker, Community Health Worker, and a Front Desk Administrative Support individual.
Our goal is to take care of students’ health by establishing trusted adult relationships to support attendance and the achievement of academic success for graduation. The GHS-SBHC offers medical visits for illness, sports and work physicals, personal health care; counseling for individuals and groups; social skills training, cognitive behavioral therapy, mindfulness techniques and anxiety reduction - with a strengths-based collaborative approach; the Sexuality and Gender Acceptance student group for LGBTQ+ students and their allies; connecting students to medical, mental, and public health resources inside the school and the broader community; Walking Club for gym credits; an education program for stress reduction with emwave stress reduction program; Youth Advisory Council leadership opportunities; and the Free Food Locker - a shelf pantry program available to all GHS students.
Must be bilingual.
30-35 hours/week
