Job Description:

Under the direction of, and working alongside, the Transplant Quality Assessment Manager, the Transplant Data Coordinator works to manage the various transplant databases including (i.e. Phoenix, United Network of Organ Sharing (UNOS), End Stage Renal Disease Registry (ESRD) as well as capturing data for other multi-organ Transplant quality improvement initiatives.

-Focuses efforts on timely and accurate data entry of clinical variables into transplant databases.

-Conducts daily clinical chart review as well as data analysis and reporting of improvement opportunities to Transplant Leadership.

-Includes capturing demographic and clinical information and reporting data to the Transplant team.

-Assists the Transplant Quality Assessment Manager with running data reports from various databases including but not limited to EPIC, SRTR and UNOS).

-Reports trended data over time with graphs and presentations. Identifies ways to enhance data capture process to optimize data integrity and increase the Scientific Registry of Transplant Recipients (SRTR) risk adjustment.

-Works with the Transplant Quality Assessment Manager to support patient quality and performance improvement initiatives.

-Must be able to coordinate data entry and analysis of multiple pathways concurrently.

-The Transplant Data Coordinator must be able to effectively communicate with all levels of personnel and function as a liaison at the department and facility levels.

-Coordinating information flow among medical, ancillary, nursing, financial and insurance staff. Identifies and reports issues to the Transplant Quality Assessment Manager for problem solving.

Essential Duties & Responsibilities

  1. Verifies information, collects, and enters all appropriate data into appropriate database (i.e. Phoenix, United Network of Organ Sharing (UNOS), End Stage Renal Disease Registry (ESRD) to keep registry current within allotted deadlines and regulatory measures

  2. Creates and maintains databases for patient management, regulatory reporting and clinical tracking

  3. Monitors non-compliance with documentation to bring to leadership to make recommendations related to area that need improvement

  4. Maintains EPIC CMS reports to ensure continuous readiness for CMS surveys.

  5. Utilizes Epic reports to identify solid organ transplant patients based on their clinical phase to automatically transfer information to UNET (Waitlist Reconciliation, Transplant Candidate Registration and Transplant Recipient Registration forms). Utilizes these tools to verify and confirm data accuracy.

  6. Develops data collection tools and quality checks to identify any discrepancies or errors

  7. Monitors and maintains accuracy and completeness of data entry to optimize data integrity

  8. Assists with the completion of UNOS staffing surveys

  9. Working with the Transplant Quality Assessment Manager, responds to specific data requests and develops data collection plans, conducts chart review, analyzes and summarizes quality improvement project data for review by the transplant team as well as key stakeholders

  10. Serves as an active participant in the Transplant continuous quality improvement work through various efforts including creation, distribution, and presentation of data

  11. Promotes and facilitates the development and identification of existing support systems necessary for improving data analysis

  12. Interacts with various members of the multidisciplinary care team to ensure accurate data capture reviewed at the Transplant Quality meetings

  13. Escalates concerns to department leadership to problem solve when needed

  14. Assists with the RCA process of patient deaths or graft failures. Notifies UNOS and completes required regulatory forms

  15. Along with the Transplant Quality Assessment Manager, serves as a Security Administrator for UNET and ESRD

  16. Attends meetings onsite as needed

  17. All other duties as assigned by the Transplant Quality Assessment Manager and Transplant Leadership

Minimum Qualifications:

  • Preferably has a clinical/medical background, i.e Licensed Practical Nurse or Medical Assistant

  • Minimum 3 years' experience in healthcare, preferably with data collection and or chart review

  • Knowledge of hospital, industry and agency standards for procedures used to collect, compile and manage/coordinate data

  • Very familiar with Microsoft Office applications
